Understanding Stroke Mechanisms: Ischemic vs. Hemorrhagic

The Mechanism of a Stroke

A stroke occurs when the blood supply to part of the brain is interrupted or reduced, preventing brain tissue from getting oxygen and nutrients. Brain cells begin to die within minutes. There are two primary categories of stroke: ischemic and hemorrhagic.

Ischemic strokes, the most common type, are caused by a blockage—usually a blood clot—in an artery leading to the brain. These can be thrombotic, where a clot forms directly in the artery, or embolic, where a clot travels from another part of the body (often the heart). Hemorrhagic strokes, conversely, occur when a blood vessel ruptures and bleeds into the surrounding brain tissue, increasing intracranial pressure and damaging cells.

The FAST Protocol: Identification and Action

  • Face Drooping: One side of the face may sag or feel numb. When the person smiles, the smile may appear uneven.
  • Arm Weakness: One arm may become weak or numb. When both arms are raised, one may drift downward.
  • Speech Difficulty: Speech may be slurred, or the person may be unable to speak or understand simple commands.
  • Time to call 911: If any of these signs are present, the window for effective treatment is closing. Immediate transport to a certified stroke center is mandatory.

Advances in imaging and surgical techniques have extended the window for thrombectomy in some patients, but speed remains the priority to ensure the salvage of the penumbra—the area of brain tissue that is threatened but not yet dead.

Risk Factors and Preventative Strategies

While some risk factors are non-modifiable, such as age, family history, and ethnicity, many are within a patient's control. Managing hypertension remains the most effective way to reduce stroke risk, as high blood pressure puts constant stress on arterial walls, making them more likely to burst or accumulate plaque.

  • Atrial Fibrillation (AFib): This irregular heart rhythm can cause blood to pool in the heart, forming clots that can travel to the brain.
  • Diabetes and High Cholesterol: These conditions contribute to atherosclerosis, the narrowing of arteries due to plaque buildup.
  • Lifestyle Choices: Smoking, sedentary behavior, and poor diet significantly increase the probability of a vascular event.
